A program developed at the University of Hawaii-Hilo's Daniel K. Inouye College of Pharmacy that helps patients manage their medications to avoid being readmitted to the hospital has achieved great success since starting its roll out in 2013.

Better Care With Transitional Med Management

Hawaii's Pharm2Pharm project is helping patients at risk of medication-related hospital re-admissions by improving medication management transitional care. Pharm2Pharm uses HCS software solutions to access prescription fill histories on their patients to create a complete and accurate medication history.

HCS Complete EHR Certification

HCS eMR version 6 achieved ONC HIT 2014 Edition COMPLETE EHR certification on May 14, 2014, which designates that the software is capable of supporting eligible hospitals with meeting the Stage 1 and Stage 2 meaningful use measures required to qualify for funding under the American Recovery and Reinvestment Act (ARRA).
