Rethinking IPFQR Reporting: From Annual Burden to Ongoing Insight

IPFQR (Inpatient Psychiatric Facility Quality Reporting) is an important part of meeting CMS requirements, but compliance should not come at the expense of valuable staff time. Facilities that do not meet applicable requirements face a 2-percentage-point reduction to their Medicare annual payment, making accurate and timely reporting essential.

For many inpatient psychiatric facilities, traditional reporting can require significant effort to collect, reconcile, validate, and prepare data for submission. Some facilities indicate the process can take 45 to 60 days of staff time. For organizations already operating with limited resources, that administrative burden can make reporting difficult to manage.

But IPFQR should be more than an annual compliance exercise. The measures are intended to help facilities assess and improve the quality of care they provide. Having access to measure results throughout the year can help organizations identify documentation and performance gaps while there is still time to address them.

A More Efficient Approach

The HCS Behavioral Health Record Compliance Toolkit streamlines IPFQR by using information already documented in the electronic health record to support measure abstraction and reporting. Facilities can review and measure results and supporting patient-level information, validate their data, and use those results to support ongoing quality improvement.

When it is time to report, HCS generates CMS-compatible submission files and submits applicable data to the CMS Hospital Quality Reporting System on the facility’s behalf. Facilities retain responsibility for reviewing their data, ensuring it is accurate and complete, and completing the required Data Accuracy and Completeness Acknowledgement (DACA).

Depending on the facility’s current process and costs, the potential annual savings can be approximately $65,000 to $75,000, in addition to avoiding the 2-percentage-point reduction to the Medicare annual payment update.

For HCS Compliance Toolkit IPFQR users, the annual report process came and went without the headaches and considerable time investment often associated with the traditional method. Equally importantly, IPFQR data was more accessible and actionable throughout the year so less time was spent assembling data and more time was available to understand what the data revealed about patient care.

From National Patient Safety Goals to National Performance Goals

The Joint Commission Transitioned from National Patient Safety Goals (NPSGs) to National Performance Goals (NPGs) for Hospital and Critical Access Hospital accreditation as of January 1, 2026. This approach brings a more finely tuned focus to pressing issues in healthcare. Although behavioral health organizations still follow the NPSGs, this change calls attention to the need for these organizations to review their current metrics, improve data collection and reporting, refine measurement-based patient care, standardize transitions, and train staff in quality improvement and performance monitoring so that they can not only answer questions about how something was done, but why it was done.
The NPGs reorganize the current Joint Commission standards and shift the focus from specific safety issues to a more comprehensive look at overall care and performance tracking, but they don’t add new accreditation requirements.

The More You Know: National Performance Goals

The NPGs organize Joint Commission standards into 14 key topics that organizations can measure and monitor over time. This change shifts focus from checklist-based compliance to performance improvement.
The Joint Commission designed the NPGs structure to improve visibility into patient safety risks, streamline evaluation processes, and support continuous improvement across care settings. The Joint Commission is interested in understanding how systems perform over time, and how leaders respond when risk and variability inevitably emerge.

How National Performance Goals Affect Behavioral Health Organizations

Behavioral health organizations continue to operate under NPSGs, which focus on patient safety-specific standards. However, the adoption of NPGs signals a future shift. Behavioral health organizations should anticipate the need for stronger coordination, consistent documentation, and forward-thinking risk management. The NPGs prioritize measurable performance in areas such as reducing suicide risk, care planning, and evaluating how care is given. Under the new guidelines, organizations must show consistent performance and measurable progress, placing greater importance on structured workflows and reliable data.
To prepare for this transition from National Patient Safety Goals to National Performance Goals, leaders can take several concrete steps now. First, review and update current outcome metrics to ensure they capture essential performance indicators. Second, pilot new or enhanced clinical documentation processes within select teams to identify gaps or challenges. Third, provide targeted staff training on quality improvement and performance monitoring practices. Taking these actions will help organizations position themselves for future accreditation requirements and drive ongoing improvement.

EHR Technology with National Performance Goals

A purpose built behavioral health electronic health record (EHR) supports organizations as they operate under the NPGs and NPSGs. HCS Behavioral Health Record helps organizations:

  • Standardize clinical documentation to support quantifiable outcomes.
  • Support patient safety and risk workflows, including suicide risk screening.
  • Align clinical processes with Joint Commission standards.
  • Maintain audit ready documentation and reporting.
  • Reduce administrative burden for clinical staff.

These capabilities are where patient centered insight meets performance driven care.

Moving Forward

The transition to NPGs indicates a broader shift in healthcare toward performance-based evaluation and data focused decision processes. Behavioral health organizations that invest in specialized EHR technology gain stronger visibility into care quality, better compliance readiness, and positive patient safety outcomes. HCS Behavioral Health Record provides the foundation organizations need to operate confidently under the NPGs.

Using AI to Capture Documentation In-Session

In many organizations, clinicians complete sessions and document hours later. That delay creates measurable operational risk:

  • Details are reconstructed from memory, reducing clinical accuracy
  • Assessment data is re-entered instead of captured once
  • Clinician dissatisfaction with time spent documenting after hours
  • InterQual criteria are missed or only partially documented
  • Claims are submitted with gaps in medical necessity
  • Denials and rework increase across the revenue cycle

With ambient listening enabled with artificial intelligence, limited studies have shown increased clinician satisfaction and reduced documentation time for both progress notes (median 27 seconds versus 128 seconds) and discharge summaries (median 114 seconds versus 459 seconds).

Patient outcome impact

  • More accurate and complete clinical records
  • Treatment plans better reflect in-session discussions
  • Reduced delays caused by authorization or reimbursement issues
  • More clinician time available for direct patient care instead of documentation

Financial / billing impact

  • Cleaner claims submission with fewer missing elements
  • Reduced denial rates tied to documentation gaps
  • Faster turnaround on appeals with supporting context already available
  • Less administrative time spent on rework across clinical and billing teams
  • Decreases loss of staff due to burnout

Capturing documentation with HCS AIDA can reduce documentation time and increase clinician satisfaction. It can positively affect patient outcomes and the financial bottom line. The HCS Behavioral Health Record includes AIDA. Capturing documentation during the session changes how both care delivery and billing function. With AIDA HCS customers can:

  • Turn conversations into structured assessments in real-time
  • Clinical criteria surfaced and validated in real time for InterQual criteria
  • Documentation more consistently supports medical necessity
  • Context for claims and appeals captured at the source

HCS Announces Availability of PIX Module for IPFQR Compliance

HCS is pleased to announce the official release of its Psychiatric Inpatient Experience (PIX) survey module, now available for all organizations using the HCS Behavioral Health EHR platform Compliance Toolkit. This new functionality is designed to help inpatient psychiatric facilities streamline compliance with the Centers for Medicare and Medicaid Services (CMS) Inpatient Psychiatric Facility Quality Reporting (IPFQR) Program while capturing meaningful, actionable patient experience feedback.

The PIX Survey, developed by CMS, is a required patient experience measure with mandatory collection beginning in 2026. It assesses key aspects of the inpatient psychiatric stay. The survey evaluates four critical domains:

  • Treatment team relationship
  • Nursing Team Presence
  • Treatment effectiveness
  • Environment

Significantly, beginning with calendar year 2026, participating facilities must administer the survey to eligible patients, typically within 24 hours prior to discharge, following CMS methodology and strict requirements for anonymity and data handling.

  • Failure to meet IPFQR reporting requirements triggers a 2% reduction to the Medicare annual payment update for inpatient psychiatric facilities

To simplify the PIX Survey process, HCS developed a fully embedded module within the framework of the EHR.

Key Capabilities of the HCS PIX Survey Module

  • Structured, user-friendly workflow to collect PIX survey responses with triggers, To Do reminders, and role specific alerts
  • Built in patient anonymity protections
  • Secure storage of completed surveys for compliance and organizational tracking
  • Interactive dashboards for trending and reviewing aggregated survey results over time
  • Support for quality improvement initiatives, patient experience monitoring, and IPFQR participation

Enhancing Patient-Centered Care and IPFQR Readiness

As CMS continues to elevate expectations for transparency and patient experience measurement in inpatient psychiatric care, these organizations need tools that make compliance efficient and meaningful. Therefore, the HCS PIX Survey module supports facilities in meeting regulatory expectations while helping leaders understand patient perception, identify trends, and implement evidence based improvements.
