Pharm2Pharm

A program developed at the University of Hawaii-Hilo's Daniel K. Inouye College of Pharmacy that helps patients manage their medications to avoid being readmitted to the hospital has achieved great success since starting its roll out in 2013.

Pharm2Pharm puts patients in direct contact with a pharmacists to ask questions about medication management and make sure their medication lists are up to date. “The hospital consulting pharmacists use risk-based criteria to identify inpatients who may benefit from further care once they leave the hospital,” said Karen Pellegrin, director of the project based in college of pharmacy’s Center for Rural Health Science. “The pharmacists also consider referrals from community doctors who have high-risk patients not currently in the hospital who might benefit.”

Once of the corners stones to the success of Pharm2Pharm has been the technology. HCS has been partnered with University of Hawaii and Hawai’i HIE since the inception.
