The Joint Commission recently released an update concerning medication reconciliation. Medication reconciliation will again be part of the RFI scored process, effective July 1, 2011. One of the largest problems with the medication reconciliation process is the difficulty in gaining accurate data when recording the patient's current medication list. HCS Medication Reconciliation offers assistance with this difficult part of the process by providing an aggregated patient medication list from insurance companies, retail pharmacies, and pharmacy benefit managers.
