Community Health Systems chooses HCS

Health Care Systems, Inc., (HCS) announces a partnership with Community Health Systems, Inc. (CHS) for HCS Medication Reconciliation.  The solution meets meaningful use requirements while centering on patient safety and improved workflow.

Medication Reconciliation at CHS

On February 24, 2011, CHS and HCS entered into an agreement to focus together on streamlining medication reconciliation at CHS hospitals.  HCS Medication Reconciliation has a proven positive impact on patient health.  With renewed focus from the Joint Commission, and with medication reconciliation as an element of meaningful use, CHS hospitals recognize the importance of medication reconciliation in providing the highest level of patient care.

CHS will use HCS Medication Reconciliation to provide the patient’s medication list upon admission.  By querying multiple prescription data sources through HCS Medication Reconciliation, CHS can verify the medications that patients actually had filled, identify critical medications (reducing errors of omission), and discover and address potential compliance issues.  CHS hospitals will also view a merged inpatient/home medication list, which will save time and increase accuracy during reconciliation.  At discharge, CHS clinicians will have the capability to quickly and effectively create electronic prescriptions, generate patient instructions and education, and automatically send information to the next care provider.

About HCS Medication Reconciliation

When a patient is admitted to the hospital, clinicians are provided, within seconds, with a detailed report of the patient’s prior medication history, including drug strength, quantity, and original fill and last refill dates. Alerts for possible medication interactions, duplicate drug therapies, compliance issues, and formulary interchanges are displayed.  During transfer and at discharge, a merged home and inpatient medication list allows for quick and efficient reconciliation.  At discharge, clinicians can create prescriptions, generate education materials, and send the discharge list to the patient and the next care provider. The Joint Commission Update on Medication Reconciliation

The Joint Commission recently released an update concerning medication reconciliation.  Medication reconciliation will again be part of the RFI scored process, effective July 1, 2011.  One of the largest problems with the medication reconciliation process is the difficulty in gaining accurate data when recording the patient's current medication list.  HCS Medication Reconciliation offers assistance with this difficult part of the process by providing an aggregated patient medication list from insurance companies, retail pharmacies, and pharmacy benefit managers.  Additionally, HCS provides all of the tools to streamline the entire process electronically.

ARH signs with HCS

Health Care Systems, Inc. is pleased to announce a partnership with Appalachian Regional Healthcare (ARH).  ARH will be working with HCS to improve medication reconciliation, pharmacy order management, and to achieve Stage 1 requirements for meaningful use. 

The ARH system has been selected as the 2010 Outstanding Rural Health Organization in the nation by the National Rural Health Association (NRHA).  It is comprised of nine hospitals operated in Harlan, Hazard, McDowell, Middlesboro, Morgan County, South Williamson and Whitesburg, Kentucky, and Beckley and Summers County, West Virginia.  ARH is accredited by The Joint Commission, and exists to improve health and promote well-being of all the people in Central Appalachia in partnership with our communities
