The solution meets meaningful use requirements while centering on patient safety and improved workflow. Medication Reconciliation at CHS On February 24, 2011, CHS and HCS entered into an agreement to focus together on streamlining medication reconciliation at CHS hospitals. HCS Medication Reconciliation has a proven positive impact on patient health. CHS hospitals will also view a merged inpatient/home medication list, which will save time and increase accuracy during reconciliation. At discharge, CHS clinicians will have the capability to quickly and effectively create electronic prescriptions, generate patient instructions and education, and automatically send information to the next care provider.
