About HCS Medication Reconciliation software Originally created in 2005, HCS Medication Reconciliation software simplifies the medication reconciliation process by providing the patient’s medication list, including prior medication fill and refill information, at the point of care within 60 seconds of the patient’s registration. The medication list is screened for interactions, duplicate therapies, and compliance issues and clinicians are alerted to these potential problems as well as any formulary interchanges at the beginning of the patient’s care. The medication fills and refill information is integral in reducing errors of omission in the medication reconciliation process. |
