(HCS) to streamline their medication reconciliation process by incorporating medication history information into the admission/registration process, and by utilizing HCS' workflow driven patient safety software, HCS Medication Reconciliation. AnMed Health will employ HCS' capability integrate with their McKesson system to achieve the goals of safe, effective user-friendly medication reconciliation within the clinician's regular work process. Additionally, HCS will link the Wellsoft Emergency Department system to the inpatient areas for medication reconciliation, eliminating the need for transcription of the medication history for patients that are admitted to the hospital from the Emergency Department.
