HCS Medication Reconciliation
HCS Medication Reconciliation automates the patient medication list retrieval process and provides a software solution to handle reconciliation at admission, transfer, and discharge.  At discharge, the patient's discharge prescription orders can be sent to a retail pharmacy.  The patient's medication list can be automatically sent to the next care provider.  HCS meets and exceeds the Joint Commission’s standard “to accurately and completely reconcile medications across the continuum of care.” Click here to learn more.
